There are many causes of a twitching belly, but in a stray dog distemper infection is most likely; seek a vet for infectious disease testing and X-rays, then treat accordingly. Pneumonia-type distemper often brings abdominal (belly) breathing from poor airflow, and distemper sequelae can cause rhythmic twitching of the belly and limbs with pain.
The photo also shows heavy eye discharge, a common distemper sign that eye drops can relieve. Prompt veterinary care is important given the risk.
