Sudden lameness is often easier to trace β€” a normal dog that suddenly limps clearly had something happen. But chronic or subtle lameness takes a careful workup. A detailed history and orthopedic exam point the vet to the right tests.

1 Sudden vs. Chronic Lameness

With sudden onset, external trauma is most likely: a paw pad thorn causing severe acute lameness; an insect bite leading to infection; a puppy that suddenly strains a cranial cruciate ligament (CCL) chasing on uneven ground, causing non-weight-bearing lameness; or an overweight dog landing badly and spraining wrist ligaments.

But a sudden limp can also mean a long-standing problem just worsened β€” a partially torn CCL becoming fully torn, or a bone tumor causing a fracture from a minor slip. Sudden lameness can also be the first sign of spinal disease (herniated disc in the neck or lumbosacral area), or disc disease / fibrocartilaginous embolism (FCE) causing rapid weakness mistaken for lameness.

Many lameness issues show in young and adult animals, improving with rest and worsening with exercise (e.g., elbow dysplasia), starting mild then progressing over weeks.

2 Early Signs Owners Notice

An obviously limping leg may not be the first sign. Early clues: reluctance with stairs, less interest in exercise, or low energy. Hip-dysplasia dogs show abnormal gait without a specific-limb limp. Subtle gait changes β€” swinging a limb instead of flexing a joint β€” or a working dog with mild exercise intolerance also count.

Orthopedic lameness can be mistaken for neurologic disease; a CCL-injured dog walks with great difficulty and weak, clumsy hind legs much like a disc-disease dog.

3 The Diagnostic Workup

A detailed history is key β€” the vet asks about age, sex, breed, general health, duration, recent change, whether it’s worse morning vs. evening or after rest vs. exercise, crying from pain, and prior treatment response.

After a general exam, the vet observes gait in the room and may review a video you bring.

Orthopedic assessment: limb inspection, palpating bone, checking muscles and joints for pain/swelling/mobility, and probing specific areas (hip/elbow/CCL disease). The neck and spine are tapped along their length; neurologic exam if suspected.

X-rays follow if bone or joint abnormality is found β€” not always needed, but they confirm suspicion and reveal exact cause for a better plan. Comparing the normal opposite limb helps subtle cases; repeat films at 4–6 weeks can show change or progression.

Biopsy or joint tap (sterile needle, with sedation) may follow X-rays. CT or MRI helps tricky cases (e.g., mild CCL tear); arthroscopy (small scope in the joint) is minimally invasive for some joint diseases.

Suspected neurologic disease: myelography, CT, MRI, spinal tap. Arthrography (water-soluble dye in the joint) maps joint extent and screens biceps-tendon issues. Bone scans have limited use in small animals.

Routine bloodwork rarely shows lameness-specific changes, but it’s useful if infection or autoimmunity is suspected, or before anesthesia for orthopedic surgery.
