What Is Canine Methemoglobinemia

Canine familial methemoglobinemia is an inherited NADH reductase deficiency causing methemoglobin buildup, bluish gums, and chocolate-brown blood. It is chronic and usually non-fatal, but oxidant exposure can cause emergencies treated with methylene blue or vitamin C.
1 The underlying cause
The root problem is a mutation in the gene controlling NADH methemoglobin reductase, an enzyme the red cells need to reduce methemoglobin back to functional hemoglobin. As a result, methemoglobin builds up. Methemoglobinemia is diagnosed when methemoglobin exceeds about 1% of total hemoglobin; at 20-50% oxygen-shortage symptoms appear, and above 60-70% circulation can fail, causing collapse, coma, and death.
2 Signs and course
The disease runs in families within certain lines and is likely autosomal recessive, affecting both sexes from soon after birth. The main sign is bluish (cyanotic) mucous membranes, and all the blood, arterial, venous, and capillary, looks very dark, chocolate-brown. At rest, temperature and pulse may seem normal. Light exercise or contact with an oxidant can trigger rapid breathing, fast pulse, weakness, unsteady gait, dullness, even fainting from lack of oxygen.
The course is slow and can last years or a lifetime; on its own it is usually not fatal unless another illness complicates it.
3 How it is identified
Three lab clues confirm it: the blood’s brown color does not turn red when shaken in air; methemoglobin level and proportion are markedly high; and red-cell NADH methemoglobin reductase activity is far below normal (under a quarter of normal). Affected dogs can show methemoglobin around 40-70 g/L, roughly a hundred times the normal value, with enzyme activity under 25% of normal.
4 Management
Because the condition is chronic and stable, many dogs need no treatment beyond the gum discoloration and brown blood. If it worsens sharply after exposure to oxidants like nitrite or nitrate, emergency reducers such as methylene blue or ascorbic acid are used. Day to day, a high dose of vitamin C can keep methemoglobin low and ease the signs.
