Sesame-sized white grains around the cat’s anus are mostly tapeworm segments, a stubborn intestinal parasite whose segments carry many eggs. On finding them, give an oral dewormer promptly; some need a repeat in two weeks until no segments remain.

After deworming give probiotics to avoid gut reaction, keep the environment clean, feed little raw meat, and deworm regularly.
