What Is Feline Idiopathic Cystitis (FIC)?

1 Overview

FIC is a diagnosis of exclusion after ruling out known causes; similar to interstitial cystitis in women; most common in young-to-middle-aged cats.

Common symptoms

Symptoms resemble other urinary diseases: more frequent urination, restlessness, blood in urine, dribbling, painful urination, inappropriate urination.

Thus the diagnosis of FIC

First rule out:

Bladder stones and urethral blockage

Urinary tract infection

Trauma

Anatomical abnormality, e.g., urethral stricture

Tumors (urinary cancer or benign)

Once all common causes are excluded, FIC is confirmed.

The cause of FIC is complex

FIC’s exact cause is unknown. Recent studies link it to neuroendocrine abnormalities: imbalance lets excitatory cortisol fail to suppress the sympathetic nervous system, increasing tissue permeability and sensory input. Pituitary/hypothalamus feedback drops, so CRF keeps releasing.

So FIC relates to the neuroendocrine system, adrenals, and bladder.

FIC recurs; reducing stress is key

Stress is a major trigger, involving neuroendocrine abnormality.

Behaviorally, chronically stressed cats are highly alert; sympathetic overreaction to aversive events changes bladder-wall integrity.

In practice, over 70% of FIC cases trace the main ‘stressor’ to conflict between housemates; other stressors: moving (3 months prior), owner changes, even rain.

Managing FIC

(1) Change diet to wet (canned/pouch) instead of dry, and increase water intake.

(2) Offer water (and food) in multiple spots; use ceramic bowls, not metal/plastic.

(3) Try the litter box in different spots and different litters; avoid noisy/busy areas.

(4) FIC is painful; during acute episodes, especially with discomfort, pain medication helps.

(5) Improve environment — reduce stress with a calming supplement; a calming tablet can ease negative mood.
