A puppy dying right after birth may be due to underlying genetic problems, or to an unsuitable environment or improper breeding. If the umbilical cord area gets infected during delivery, the puppy may develop a systemic inflammatory response and die.
Also, if secretions in the mouth and nose aren’t cleared promptly after birth, breathing is easily affected and the puppy can suffocate.
When my breeder friend lost a newborn pup it was an unclean delivery and a blocked airway, and the vet stressed clearing the mouth and a warm clean box. I'd have the basics ready and watch mom closely, because those first minutes decide a lot.
