Spaying a female cat at a proper hospital causes no real harm to the cat. There is some surgical risk, mainly from anesthesia, so pre-operative evaluation of the cat’s health and anesthesia suitability is needed. With a successful surgery and good care, the wound heals in about a week.

By contrast, not spaying seriously harms the cat; repeated heat cycles raise the risk of reproductive diseases like pyometra and ovarian tumors. Spay surgery is now very mature with low risk, so don’t avoid it out of fear – take the cat to a proper hospital, which also lowers the risk.
